Max Care’s nursing services are designed to help the client recover from a recent illness. Many ongoing health conditions can cause complications during the recovery process, therefore the plan of care will cover the most recent diagnosis and an overall assessment of well-being and goals to be directed towards a full recovery. For different risk factors that are presented, there are many options like therapy, social workers or local townships, or other health care resources.
For physicians, we also have case management while the patient is at the hospital, recovering from a procedure or illness. The case manager RN can visit the patient at their home to answer questions prior to their medical treatment. They will also be able to help coordinate any other non-medical needs that the patient has including a home health aide.
